During this rotation students can expect to see patients for chronic care management, acute sick visits, walk ins, pap smears, see women who are HIV positive who are in their 1st, 2nd and 3rd trimesters, clinical breast exams, preventative care for women 40 and up, contraception, ect. Emphasis on prevention of disease and/or risk reduction, shared patient-caregiver-provider decision-making, in addition to age-appropriate screenings, comprehensive chronic disease management, personalized health education and care plans, preventative counseling. Please note: This practice serves primarily patients in the LGBTQ+ identifying community, and in a culturally, ethnically, socioeconomically diverse environment. The student must be able to be respectful and careful not to lose the trust and respect of the patients. I do approximately 40% telehealth (on Wednesday and Friday mornings/afternoons), and 60% in-person encounters (mainly during Wednesday evenings and every other Saturday).

I practice in an inpatient advanced heart failure and cardiology service that manages adults hospitalized with acute and chronic heart failure, volume overload, cardiogenic shock risk, and multi‑system complications related to cardiac dysfunction. Our setting provides students with exposure to high‑acuity cardiovascular care within a collaborative, fast‑paced hospital environment. The inpatient service focuses on: -Comprehensive heart failure management for patients with reduced or preserved EF -Initiation, titration, and monitoring of guideline‑directed medical therapy (GDMT) -Daily rounding, clinical assessment, and interdisciplinary communication -Diagnostic interpretation including labs, ECGs, telemetry, chest imaging, echocardiograms, and volume‑status indicators -Heart failure education, discharge planning, and transitions‑of‑care management -Device‑supported care exposure, including CRT, ICDs, CCM, Barostim, and pulmonary artery pressure monitoring as clinically appropriate Students gain hands‑on experience with: -Inpatient clinical decision‑making -Fluid management strategies -Symptom decompensation evaluation -Medication adjustments in real time -Patient and family education -Collaboration with cardiology, hospitalists, case management, nursing, and ancillary teams This setting is ideal for students seeking strong exposure to acute care, cardiology, complex comorbidities, and evidence‑based inpatient heart failure management. I welcome motivated learners who are engaged, curious, and ready to develop advanced assessment and clinical reasoning skills at the bedside.

Island City Urgent Care & Medical Center (google and call to contact) has openings for Family Medicine and AG-ACNP students to join a small practice in Wilton Manors FL. No cost and low patient volume clinic with providers who are willing to teach only to students willing to learn. Most clinical rotations students have gone to they are wall flowers and are a phycicians glorified scribe, not here. The expectation is that you are in the hot seat and you should do every visit from top to bottom and documenting in the EHR, assessing every patient, prescringing and treating, performing all skills. Roughly 8-15 patients daily 10-7pm awards pleanty of time to chart and lots of time for learning. These patients are treated different in that some do not have or could not get into their primary care provider but when assessed and treated here a full assessment is performed on all new patients, not a focused assesment as seen in most other urgent cares. We have a large family medicine componant that is fed off of the urgent care and the patients often begin doing their primary care services because of our walk-in element of care. You will be taught about x-ray, ultrasound, pulmonary function testing, and laboratory services done on site. After assessment of the patient you will present to your preseptor your findings and plan and if agreed upon deliver that treatment plan to the patient and render care under the guidance of the preceptor. We know what it's like to go through school and we want to teach the next generation to be better and we teach what we wish we would have been taught so you will succeed. Business attire required with no scrubs and a white coat. You can be late but you can't go home early because charting must get finished. It's a very relaxed environment but best to be reserved for a later clinical site and not the first one after you have developed or need to develop how to formulate a plan. Every RN can assess and chart on the patient but bring your resources so you can understand how to formulate and prescribe a plan quickly as is the expectation with any job by any provider.
